Purpose

To evaluate the real-world diagnostic performance of the ISOLD score-a probabilistic formula based on absolute intraocular IL-10 and IL-6 levels-and to compare it with absolute cytokine thresholds and the IL-10/IL-6 ratio for vitreoretinal lymphoma (vitreoretinal lymphoma) diagnosis.

Methods

One hundred sixty-six patients who underwent intraocular sampling for IL-6 and IL-10 measurement at a tertiary vitreoretinal lymphoma referral center between January 2017 and June 2019, yielding 183 samples (156 aqueous humor from anterior chamber paracentesis and 27 vitreous).

At a threshold of 0, the ISOLD score yielded overall sensitivity of 86.8%, specificity of 98.6%, PPV of 94.3%, and NPV of 96.6% across all 183 samples.

The IL-10/IL-6 ratio at a threshold of 0.6 in aqueous humor (n=150) demonstrated sensitivity 77.8%, specificity 93.9%, PPV 63.6% and NPV 96.9%; at a threshold of 1 in vitreous (n=27), sensitivity, specificity, PPV, and NPV were 80%, 100%, 100%, and 63.6%, respectively.

The ISOLD score is a reliable tool for vitreoretinal lymphoma diagnosis in routine clinical practice, with performance validated primarily on aqueous humor samples obtained by anterior chamber paracentesis.

It outperforms the IL-10/IL-6 ratio alone by providing a continuous probabilistic estimate. False-negative results may occur in immunosuppressed patients or those receiving corticosteroids or chemotherapy.
