Background

The utilization of mechanical circulatory support (mechanical circulatory support) in patients presenting with ST-elevation myocardial infarction (ST-elevation myocardial infarction) complicated by cardiogenic shock (cardiogenic shock) has increased substantially in recent years.

Purpose

To assess the outcomes of patients with ST-elevation myocardial infarction and cardiogenic shock based on availability of mechanical circulatory support devices.

Methods

Were stratified by the level of mechanical circulatory support received, and comparisons were made across hospital types.

n = 260 patients

A total of 19, 260 patients with ST-elevation myocardial infarction and cardiogenic shock were identified during the study period.

Among patients who received intra-aortic balloon pump only, those admitted in centers with only intra-aortic balloon pump had the shortest time to mechanical circulatory support initiation compared to other centers.

Similarly, among those who received Impella, time to mechanical circulatory support was shortest in centers with Impella only compared to those with veno-arterial extracorporeal membrane oxygenation and transplant centers.

Among patients who received intra-aortic balloon pump only, there was no significant difference in mortality based on the location of hospitalization.

In patients with ST-elevation myocardial infarction complicated by cardiogenic shock, outcomes vary not only by the type of mechanical circulatory support utilized but also by the mechanical circulatory support capabilities of the treating hospital.

These findings suggest that institutional expertise and resource availability may play a significant role in influencing both in-hospital and short-term outcomes, even among patients receiving the same level of circulatory support.
