Purpose

To investigate the structural and vascular alternations of optic disc melanocytoma (optic disc melanocytoma) using optical coherence tomography (optical coherence tomography) and optical coherence tomography angiography (OCT angiography).

Methods

SUBJECTS: This study included 34 eyes diagnosed with optic disc melanocytoma and 15 eyes with juxtapapillary choroidal melanoma (choroidal melanoma) who referred to Beijing Tongren Hospital from July 2019 to March 2025.

n = 34 eyes
Results

peripapillary hyperreflective ovoid mass-like structures found around the optic nerve

88.2%
ODM eyes
17.1%
Fellow eyes
More results

Epiretinal membrane was present in 20.6% of optic disc melanocytoma cases, compared to fellow eyes (2.94%, P = .054).

More results

OCT angiography revealed reduced optic nerve head vessel density in optic disc melanocytoma eyes, predominately affecting the radial peripapillary capillary layer on the tumor surface, which resulted in 14 cases of localized radial peripapillary capillary atrophy and patchy nonperfusion areas on En-face images.

Conclusion · 1 of 3

Optic disc melanocytoma is associated with significant structural and vascular alterations, including PHOMS, optic nerve head hypoperfusion, and eventually retinal nerve fiber layer atrophy.

Conclusion · 2 of 3

These features reflect both mechanical compression and ischemic injury to the optic nerve and can be used to assess visual prognosis.

Conclusion · 3 of 3

The absence of intrinsic tumor vasculature in optic disc melanocytoma underscores its fundamental difference in biological behavior from juxtapapillary choroidal melanoma.
