Purpose

To validate the visual, refractive and higher-order aberration outcomes of a simplified ray-tracing-guided LASIK adjustment strategy in a large real-world cohort.

Methods

This retrospective single-arm study included 117 consecutive patients (234 eyes) who underwent ray‑tracing‑guided LASIK between December 2024 and December 2025.

Vector analysis showed excellent astigmatism correction accuracy.

Total HOAs, spherical aberration, and coma showed no statistically significant differences compared with preoperative values, whereas trefoil aberration was associated with a significant postoperative decrease.

Multiple linear regression revealed that higher postoperative spherical aberration and trefoil were associated with worse uncorrected distance visual acuity.
