Aberration-guided Intraocular Lens Power Calculation in Keratoconus: Ray Tracing Optimization.

Journal of cataract and refractive surgery · 2026
Purpose

To evaluate the accuracy of ray tracing optimization (ray tracing optimization) for predicting postoperative refractive error in keratoconic cataract eyes.

Methods

Case series.

Results

predicted glasses prescription was accurate, off by no more than 1 diopter, for most eyes

86.3%
SEQ (±1.0D)
81.8%
J0 (±1.0D)
77.3%
J45 (±1.0D)
More results

Bland-Altman analysis showed good agreement between ray tracing optimization-predicted and postoperative subjective refraction, particularly for spherical equivalent (spherical equivalent) and J0 (with/against-the-rule astigmatism).

Cataract surgery in keratoconic eyes did not induce significant variations in the corneal Zernike coefficients.

More results

Ray tracing optimization accuracy was highest when vertical coma (Z3-1) predominated as the main higher-order aberration.

Conclusion · 1 of 2

Aberration-guided ray tracing optimization provides accurate postoperative refractive predictions in keratoconic cataract eyes.

Conclusion · 2 of 2

Accuracy is particularly strong in eyes dominated by vertical coma, supporting inclusion of measured higher-order aberrations in intraocular lens power selection.
