Background

The arthroscopic modified Bröstrom procedure (AMBP) is widely used to treat chronic lateral ankle instability (chronic lateral ankle instability).

Purpose

To introduce all-inside arthroscopic lateral capsular shift (arthroscopic lateral capsular shift) and compare outcomes with those for AMBP.

Methods

We retrospectively analyzed 132 patients (139 ankles) with chronic lateral ankle instability who underwent all-inside arthroscopic procedures between November 2017 and December 2022 and had a minimum follow-up of 24 months: 88 ankles with good ligament remnants underwent AMBP, and 51 ankles with poor or absent ligament remnants underwent arthroscopic lateral capsular shift.

For both groups, functional scores improved significantly after surgery, with no significant differences between groups in final scores or the proportion of patients who achieved functional score improvements exceeding the minimal important change (all P > .05).

Final radiological parameters and degree of correction were also comparable between groups.

Our study showed that arthroscopic lateral capsular shift achieved clinical and radiological outcomes comparable to those of AMBP, with low recurrence rates after at least 2 years of follow-up.
