Background

Postoperative hemorrhage (postoperative hemorrhage) is a serious complication after supratentorial brain tumor resection.

Purpose

This study aimed to evaluate the associations of neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, and systemic inflammation response index with postoperative hemorrhage and to develop an integrated early postoperative risk stratification model.

Methods

In this retrospective cohort study, patients with > 48 hours of neurosurgical intensive care unit stay after elective supratentorial tumor resection were temporally divided into development and validation cohorts.

The study included 587 patients (438 in development, 149 in validation).

Postoperative hemorrhage occurred in 32.9% (144/438) and 32.2% (48/149), respectively.

Systemic inflammation response index showed the highest AUC (0.686) compared with neutrophil-to-lymphocyte ratio (0.673) and platelet-to-lymphocyte ratio (0.669).

Adding systemic inflammation response index to the clinical baseline model provided significant incremental value (IDI 0.043, P < 0.001; event NRI 0.208, P = 0.011).
