Background

The epidemiological features of orbital apex syndrome are limited.

Purpose

Therefore, this study aimed to present the clinical features and determine the causes to evaluate the visual prognosis of patients with orbital apex syndrome.

Methods

The final cohort comprised 66 patients with orbital apex syndrome (mean age: 54.95 ± 18.48 years; male: 36.36% and female 63.64%).

All patients presented with unilateral involvement: 25 with right orbital apex syndrome and 41 with left orbital apex syndrome.

However, the initial ocular presentations varied: blurred vision was reported in all patients, with 63.64% of them suffering from severe visual function impairment (best-corrected visual acuity, BCVA < 20/200), followed by ophthalmoplegia in 56 patients (84.85%), and headache/facial numbness in 40 patients (60.61%).

The clinical manifestations and etiologies of orbital apex syndrome are heterogeneous. Patients with orbital apex syndrome suffer from severe impairment of visual function, even after undergoing aggressive and comprehensive treatment.

Our findings indicated that the inflammation was the most frequent cause of orbital apex syndrome and correlated with better visual outcomes. CLINICAL TRIAL NUMBER: Not applicable.
