Purpose

To characterize optical coherence tomography (optical coherence tomography) features of vitreoretinal lymphoma (vitreoretinal lymphoma) at baseline, identify features associated with visual prognosis, and describe longitudinal optical coherence tomography changes following intravitreal methotrexate (intravitreal methotrexate) treatment.

Methods

Were stratified as isolated vitreoretinal lymphoma or vitreoretinal lymphoma with concurrent central nervous system involvement (vitreoretinal lymphoma-CNS).

Sub-RPE deposits were the most prevalent optical coherence tomography feature (92.1%), followed by subretinal deposits (65.8%) and intraretinal deposits (23.7%).

In the multivariable generalized estimating equations model, baseline BCVA ( p = 0.009) and ellipsoid zone disruption ( p = 0.026) were associated with worse visual outcome.

Twelve eyes of 8 patients comprised the intravitreal methotrexate longitudinal subgroup (mean 7.7 ± 0.8 injections).

Optical coherence tomography features showed that vitreous cells resolved rapidly, sub-RPE and subretinal deposits regressed slowly, whereas ellipsoid zone disruption remained unchanged.

Sub-RPE deposits are the most prevalent optical coherence tomography finding in vitreoretinal lymphoma. ellipsoid zone disruption and ellipsoid zone-RPE attenuation are associated with worse visual prognosis.

Following intravitreal methotrexate treatment, optical coherence tomography reveals differential response patterns across lesion compartments, supporting quantitative optical coherence tomography assessment as a sensitive, noninvasive tool for treatment monitoring.
