Patients with a low PreMiCC score had a higher rate of mild Crohn's disease.

External validation of a prediction score for identifying a mild course of Crohn's disease.

Diogo Couto Sousa et al. · European journal of gastroenterology & hepatology · 2026
Background

Crohn's disease has a heterogeneous course.

Methods

Were classified as having a score ≤2 or >2.

n = 342 patients
Results

Low PreMiCC score patients had a mild Crohn's disease course more often (42% vs 17%).

More results

At diagnosis, 55% had a PreMiCC score ≤2.

The PreMiCC score independently predicted biologic therapy [odds ratio = 1.65, 95% confidence interval (CI) = 1.33-2.04; P < 0.001].

Time to treatment escalation was shorter in patients with a score >2 (log-rank P < 0.001).

“
Conclusion

This first external validation confirms the predictive value of the PreMiCC score in a real-world cohort, supporting its use as a simple tool for early risk stratification in Crohn's disease.
