Among 108,597,869 patients, 146,356 had sarcoidosis, and 5,609 had sarcoid uveitis, with mean ages of 62.6 years and 58.8 years, respectively, and female predominance in both cohorts (56.5-60.4%).

Most sarcoidosis patients were White (53.4%), while the majority of sarcoid uveitis patients were Black or African American (43.68%).

Almost 4% of patients with sarcoidosis developed uveitis within 5 years, and the incidence of uveitis in sarcoidosis patients remained relatively constant, while the prevalence of both sarcoidosis in our cohort and uveitis in sarcoidosis patients increased in our study period.

Mean age was 51 years.

The cohort was predominantly female (21,894, 57%), non-Hispanic (27,915, 72%), and White (20,467, 53%).

Inflammatory spondyloarthropathy (SpA) showed the greatest systemic diagnosis outcome (8.62%) while herpes viruses were the predominate infectious etiologic diagnosis (8.02%).

Etiologies varied by anatomy with inflammatory SpA demonstrating the greatest cumulative incidence among anterior uveitis (34.14%), multiple sclerosis displaying a high cumulative incidence among intermediate uveitis (7.23%), and lymphoma concentrating in posterior/panuveitis (0.35%-5.22%).

The 5-year cumulative incidence of any uveitis among patients with optic neuritis in patients with MS was 0.986% (95% CI 0.846-1.15) compared to 0.543% (95% CI 0.437-0.676) in those without optic neuritis (log-rank P < 0.0001), with a HR of 1.95 (95% CI 1.51-2.53).

Intermediate uveitis demonstrated the strongest associations with optic neuritis across study groups.
