Background

The impact of atrial fibrillation (atrial fibrillation) on outcomes of endovascular thrombectomy (endovascular thrombectomy) in patients with acute ischemic stroke (acute ischemic stroke) caused by large vessel occlusion (large vessel occlusion) has been studied, with no definitive characterization of how atrial fibrillation might influence the technical and clinical outcomes of endovascular thrombectomy.

Purpose

We aimed to perform a meta-analysis to assess the effects of atrial fibrillation on the outcomes of endovascular thrombectomy for acute ischemic stroke patients.

Methods

We searched PubMed, Scopus, and WOS till May 2026 for studies comparing outcomes of endovascular thrombectomy for acute ischemic stroke among patients with and without atrial fibrillation.

n = 16,094 patients

Results

Atrial fibrillation patients less likely to reach good functional recovery after thrombectomy

We included 19 studies comprising 16,094 patients (6,283 with atrial fibrillation versus 9,811 without atrial fibrillation).

Mortality was significantly higher in the atrial fibrillation group (OR = 1.3, 95% CI 1.14-1.63, p < 0.01).

Current observational evidence suggests that endovascular thrombectomy may remain effective and safe in patients with atrial fibrillation. However, the certainty of evidence is very low, and the findings should be interpreted cautiously.
